Constitutional Hydrotherapy: A Foundational Naturopathic Modality

Sometimes described as temperature-based circulatory therapy, this method is rooted in the original naturopathic constitutional hydrotherapy technique.
If you’re familiar with the idea of hydrotherapy you probably think of hot tubs, saunas, pools and basic contrast baths. That’s not what we’re talking about here. The type of hydrotherapy I want to spotlight is the original naturopathic version known as constitutional hydrotherapy. It was refined in the early 20th century, deeply tied to classical naturopathic medicine, and is still offered here at our clinic in Portland.
What Is Constitutional Hydrotherapy?
At its core, constitutional hydrotherapy uses alternating hot and cold compresses placed on the torso (typically chest, abdomen and back) combined with very gentle physiotherapy devices (electrical sine-wave impulses, diathermy, etc.).
Dr. Otis G. Carroll not only coined the term but wrote that the therapy “changes the very constitution of each individual cell”. Hydrotherapy treatments help stimulate the body’s own healing mechanisms by improving or restoring normal organ function through the circulation. It can help a wide variety of conditions. It is completely drugless, safe for all ages, even babies and expecting mothers.
It works by improving and moving the circulation, it's the blood and oxygen as well as the clean-up crew cells that needs to moved in order to maximize function and healing.
Why It Matters in Functional Medicine & Naturopathic Practice
In functional and restorative medicine we focus on root causes, systems interactions, and optimizing physiology rather than just suppressing symptoms. Constitutional hydrotherapy aligns perfectly with that ethos. Here’s how:
It boosts circulation and lymphatic flow so organs of detoxification (liver, kidneys, skin, lungs) get the blood and oxygen they need to function.
It supports the autonomic nervous system (especially parasympathetic—rest and repair) by the hot-cold reflexive changes and the electrical stimulation.

What to Expect in a Session
Here’s a breakdown of how the treatment works in practical terms:
Each treatment is about a 45-60 minute session (many clinics use this timeframe and the original Carroll versions were built around that).
A brief intake or check of vital signs, maybe questions about your current state of health and your goals.
You lie down, typically from neck to pelvis (torso bare or draped in a comfy robe) on a treatment table. Warm towels are applied to the front or back of torso, followed by cool towels, in a timed pattern.
At the same time, one or more physiotherapy devices may be used: sine-wave stimulation on the back or abdomen, diathermy for deeper heat, or high-frequency current in acute inflammatory or musculoskeletal applications.
At the end of the sequence a gentle rub or friction massage may be done to revive and set the shift. Some patients report it feels deeply relaxing and rejuvenating.
After the session you are encouraged to rest and hydrate. Because circulation and lymphatic flow have been stimulated your body is primed for repair.

Some texts list pregnancy or cancer as relative contraindications but many experienced naturopaths use protocols safely in those scenarios when individually tailored.
