Functional Lab Testing for Hormones, Gut Health, and More: What's Available Without a Doctor's Visit

woman sleeping with pillow over her head

We see a particular kind of patient pretty regularly. Usually, they've been to their GP, maybe a gastroenterologist, an endocrinologist, possibly a rheumatologist. Every specialist rules out the thing they were looking for - which is genuinely good news - but nobody has actually explained why this person still feels terrible. Tired, bloated, anxious, wired at night, foggy during the day, gaining weight for no clear reason. All the standard labs came back as "normal" or "unremarkable".

Here's the thing to understand about standard labs: they're designed to catch disease, not to assess how well your body is actually functioning. There's a difference between "your thyroid isn't diseased" and "your thyroid is working optimally.", also between "you're not anemic" and "your iron stores are robust enough to support your energy." Conventional medicine is really good at ruling out the big, scary things, and we can do that too when it's needed, but our treatment focus is from a totally different angle.

Think of it less like an ER visit and more like a preventative visit to a good mechanic for your car where they change the oil, check the spark plugs, the tire pressure, rotate the tires, put in the right fuel, vacuum the interior, etc. These practices help the vehicle run smoothly for longer and can go a long way in keeping a check engine light from popping up unexpectedly.

That's where functional lab testing comes in.

Here's the thing to understand about standard labs: they're designed to catch disease, not to assess how well your body is actually functioning.

What Is Functional Lab Testing?

test tube in a lab

Functional labs look at the body through a different lens than standard blood work. Instead of asking "is this a disease?", we're asking "is this optimal?" And more importantly, why isn't it? These tests can assess hormone production and metabolism, nutrient status down to the cellular level, gut microbiome health and digestive function, toxic load from mold, heavy metals and chemicals, adrenal function and stress response, and more.

Many of these tests aren't available through a standard doctor's office visit, or if they are, they may not be interpreted with much depth. We offer most of them as direct-to-consumer kits you can order online, but we also utilize these same tests every day with our patients in-office. You can collect your sample at home, and have results delivered securely. Current pricing is listed on each individual test page. Anyone in the US can order. Oregon and Washington residents can also establish full naturopathic care with us, but wherever you are, a functional wellness consultation is available to help you actually understand what your results mean.

Hormone Testing

Hormones are one of the most common things we investigate, and one of the most commonly missed by standard panels. A TSH alone doesn't tell you much. Estradiol on a random day of the cycle tells you almost nothing. This test is best for anyone dealing with fatigue, mood changes, irregular cycles, PMS, perimenopause symptoms, low libido, sleep problems, or unexplained weight changes.

DUTCH Complete (urine, at-home kit) is our most comprehensive hormone panel. DUTCH stands for Dried Urine Test for Comprehensive Hormones, and it goes well beyond a standard blood draw, measuring not just hormone levels but how your body is actually metabolizing and clearing them.

Two people can have the same estradiol level and have completely different hormone-related symptoms based on how their body is processing it. The DUTCH test also includes cortisol rhythm, which gives a picture of adrenal function throughout the day, something a single morning cortisol draw can't do. This test is ideal for women with PMS, perimenopause, painful or heavy periods, PCOS, suspected estrogen dominance, and men dealing with fatigue, low testosterone, or mood changes.

For a simpler hormone snapshot - particularly useful for perimenopause, cycle tracking, or a baseline hormone picture we utilize the Saliva Hormone Wellness Panel (Doctor's Data) measures estrogen, progesterone, testosterone, and DHEA from saliva collected at home.

Adrenal Testing

Adrenal fatigue is one of those terms that conventional medicine providers usually roll their eyes at. They are trained to only recognize full-blown Addison's disease, so anything less than that tends to get dismissed. But the spectrum between "totally fine adrenals" and "clinical adrenal insufficiency" is very wide, and a lot of people live in that middle zone for years without answers.

Chronic stress, poor sleep, burnout, that wired-but-tired feeling, the need for coffee just to be functional... these are worth investigating.

The Comprehensive Adrenal Function Test (Doctor's Data, saliva) measures cortisol at four points throughout the day plus DHEA, giving a real picture of your diurnal cortisol rhythm and adrenal reserve. This is far more informative than a single morning blood draw.

Gut Health and Digestive Testing

The GI not only helps break down our food and absorb nutrients, it also produces neurotransmitters regulating our mood, processes and recycles hormones and controls much of our immune system function. Bloating, constipation, loose stools, food reactions, skin issues, mood problems, autoimmune flares can all be affected by the microbiome. We see a lot of patients who've had a colonoscopy and/or endoscopy that's normal, Celiac disease has been ruled out, and they are usually told their digestion is fine or it's just "IBS". But what they actually need to know is what's living in there and how well they're digesting their food.

The GI Advanced Comprehensive Digestive Test (US BioTek) is the stool panel we use most often. It assesses digestive function, absorption, microbiome diversity, beneficial and pathogenic bacteria, yeast overgrowth, parasites, and inflammatory markers. We've done a full comparison of this test vs. the GI-MAP if you want to get into the weeds, you can read that here.

Food Sensitivity Testing

healthy plate of food

There's a big difference between a food allergy (think hives, anaphylaxsis) and a food sensitivity. Sensitivities are hard to track on your own, because the reaction can happen so far removed from the meal that caused it, sometimes by 2-3 days. It can show up as bloating the next morning, a headache after a meal two days ago, or brain fog that seems to come and go a randomly. Digestive symptoms like gas, chronic constipation, diarrhea and skin rashes like eczema can all be driven by food sensitivites as well.

The US BioTek 96 Food Sensitivity Panel (blood, at-home finger prick) tests IgG reactions to 96 common foods and can help identify foods that are driving chronic symptoms and digestive inflammation.

Nutrient Testing

hand with vitamins

You can eat a very healthy diet and still have functional nutrient deficiencies such as poor absorption, poor conversion or metabolism of certain nutrients, and even chronic stress depleting key nutrients. Medications can affect nutrient uptake as well. Standard blood levels can screen for deficiency or excess, but they don't tell you how well nutrients are actually being utilized at the cellular level.

The Metabolomix+ (Genova, urine at-home) is a comprehensive organic acids and nutritional assessment that looks at B vitamins, amino acids, fatty acids, antioxidants, mitochondrial function markers, and more. It's particularly useful for people with chronic fatigue, anxiety, mood changes or poor immunity.

For mineral and heavy metal status specifically, Hair Heavy Metal and Mineral Testing (Doctor's Data) is an easy, non-invasive, at-home collection that gives a window into long-term mineral status and toxic metal accumulation. We often recommend this for kids or those who hate blood draws!

Environmental Toxin Testing

mold

Our toxic load can be a driver of many chronic symptoms and diseases. Mycotoxin/Mold exposure, pesticide accumulation, phthalates and bisphenols from everyday plastics, volatile organic compounds from our furniture, clothes and more. These are common, often invisible, and can cause fatigue, hormone disruption, neurological symptoms, and immune problems.

The EnviroTox Complete + Metals (Mosaic Diagnostics, urine) is one of the most comprehensive toxin panels available - over 127 analytes covering mycotoxins, heavy metals, pesticides including glyphosate, phthalates, bisphenols, VOCs, and more. If there's a history of water damage in a home, occupational exposure, if you live near a farm that uses pesticides and herbicides or just have symptoms that have never had a good explanation, this is worth considering.

Thyroid Testing

We've written about this before, but a TSH alone is not a proper thyroid panel, it's a single screening test. Lots of people have normal TSH and still have hypothyroid symptoms due to a number or reasons, some of the most common we see include:

  • Because the conversion of T4 to active T3 is the problem

  • Because autoimmune antibodies are attacking the gland, destroying it's function slowly

  • Because the adrenal glands are so taxed it is putting additional strain on the thyroid

  • Because of sub-optimal iodine levels

If you've been told your thyroid is "fine" but still have symptoms such as cold hands and feet, weight gain or inability to lose weight, brain fog, constipation, or hair loss, a full thyroid panel is needed.

The Comprehensive Thyroid Test (Quest, blood draw at a local lab) includes TSH, Free T4, Free T3, Reverse T3, and thyroid antibodies (TPO and TgAb).
